Becoming an Occupational Therapist in Phoenix, AZ

In order to be an occupational therapist, the Arizona Department of Health Services ( requires the following:

  • Good moral character; not convicted of misdemeanor or felony
  • Successfully completed the academic requirements for occupational therapy
  • Successfully completed a supervised fieldwork training/experience of 920 hours
  • Pay the application fees
  • Pass the certification exam
